การปลูกถ่ายตับ (อังกฤษ: liver transplantation) คือการเปลี่ยนตับที่เป็นโรคด้วยตับที่ปกติ ส่วนใหญ่เป็นการปลูกถ่ายโดยผ่าตัดเอาตับเดิมออกและใส่ตับใหม่ลงแทนที่ในตำแหน่งเดิม ปัจจุบันการปลูกถ่ายตับเป็นวิธีรักษาโรคตับระยะสุดท้ายและตับวายเฉียบพลันที่ได้รับการยอมรับเป็นอย่างดี และยังเป็นการรักษาที่มีค่าใช้จ่ายมากที่สุดชนิดหนึ่งในการแพทย์แผนปัจจุบันอีกด้วย การผ่าตัดเพื่อปลูกถ่ายตับนั้นใช้ทรัพยากรมากและใช้เวลาตั้งแต่ 4-18 ชั่วโมง ขึ้นกับผลการรักษา โดยจำเป็นต้องมีการตัดหลอดเลือด เชื่อมต่อหลอดเลือด เชื่อมเนื้อเยื่อของตับและช่องท้องเป็นจำนวนมาก และจำเป็นต้องมีการคัดเลือกผู้ที่จะรับการปลูกถ่ายเพื่อเปรียบเทียบความเข้ากันได้ของเนื้อเยื่อเป็นอย่างดี
